Manual therapy refers to specific mobilization and manipulation techniques applied entirely by the physiotherapist's hands to restricted or painful joints, without using any devices or machines. Current clinical guidelines in medical literature (Cochrane, PEDro) strongly support (Level A evidence) manual therapy as a primary conservative approach for neck hernia, lumbar hernia, facet syndrome, and frozen shoulder syndrome.

At the mechanical level, joint fluid (synovial) dynamics that are disrupted in diagnosed conditions are stimulated. In spinal hernias, specific traction and gliding techniques are applied to the facet joints to reduce mechanical pressure (compression) on the nerve. This triggers the dissipation of inflammatory fluid around the tissue and restores fresh blood/oxygen flow to the region.

At the neurophysiological level, the real healing mechanism is activated (Bialosky Model). Rhythmic and specific pressures applied to the joint send new electrical impulses to the brain via the spinal cord. When the brain receives these impulses, it activates descending pain inhibition pathways and triggers the release of the body's natural painkillers (endorphins, serotonin). In our clinic, completely customized, safe, and international-standard manual therapy concepts (Mulligan, Maitland, Kaltenborn) are applied to physician-approved clients.

Myofascial Release (Fascia Therapy)

Fascia is a three-dimensional web-like connective tissue that wraps around muscles, nerves, and organs throughout the body. Inactivity, poor posture, or micro-traumas lead to adhesions and tension in the fascia. With myofascial release techniques, the physiotherapist targets these adhesions using specialized hand manipulations. The goal is to restore elasticity and glide to the fascia, increase range of motion, reduce muscle tension, and accelerate regional blood flow.

Trigger Point Therapy

Trigger points, commonly known as 'muscle knots', are painful nodes that become oxygen-deprived and nutrient-deprived as a result of muscle fibers remaining constantly contracted. This condition creates pain not only at that point but also in wide areas through referred pain, such as headaches or arm/leg aches. In trigger point therapy, the physiotherapist applies specific pressure (ischemic compression) to release these knots. Blood flow is temporarily cut off by pressure; when released, fresh blood and oxygen rush into the area, supporting rapid muscle relaxation and pain relief.

Graston Technique & Instrument-Assisted Soft Tissue Mobilization (IASTM)

The Graston technique is a modern soft-tissue treatment applied with patented stainless steel instruments specifically designed for different body contours. This technique is used to deeply scan and treat fascial restrictions, scar tissues, and chronic soft tissue injuries. The instruments are slid over the tissue to detect the location of adhesions and initiate micro-circulation, triggering the body's natural healing mechanisms.

Joint Mobilization & Manipulation

Applied within the framework of world-class concepts such as Maitland, Mulligan, and Kaltenborn, these techniques aim to release restricted joint movements. Mobilization refers to rhythmic, slow gliding and traction movements applied to joints within painless limits by the physiotherapist. Manipulation, commonly known as 'joint cracking', consists of high-velocity, low-amplitude (HVLA) thrust techniques performed in suitable cases to widen the joint space and block pain signals by stimulating intra-joint receptors.

No, manual therapy is a painless method. Manipulation techniques, commonly known as 'cracking', are only performed on suitable cases and when deemed necessary. The primary goal is to relax the tissue around the joint with slow, rhythmic movements and safely increase joint mobility.

On the contrary, in hernia cases diagnosed by a specialist physician, manual therapy is one of the evidence-based primary applications in international guidelines. With correct techniques, it physically helps reduce the pressure (compression) on the nerve.

The number of sessions varies depending on whether the problem is acute (recent) or chronic (long-term). Relief is usually felt in the first few sessions; however, for permanent functional gain, an average protocol of 4 to 8 sessions is recommended based on the client's condition.
